Please arrive at least 15 minutes prior to your appointment in order to complete the registration process. It is imperative you complete the new patient forms prior to arriving to your scheduled appointment. These forms request detailed information that requires time to complete. If you arrive at your appointment without these forms complete, you maybe asked to reschedule your appointment in an effort to stay on schedule. Any applicable co-payment will be collected at that time. Ideally wear comfortable clothes and be prepared to get into a gown so that a complete physical exam can be performed during your consultation. If you have compression hose, please bring them with you so we may inspect them.
